of our Lighthouse Private Christian Academy Family!

Please make certain to Open the Family Handbook and the Photo Release form below.
Open the Exceptional Student Form if it applies to your student.

Print, Read and Sign Page 50 and 51 of the Family Handbook,
Fill out the Photo Release Form and the Exceptional Student Agreement Form (if it applies).
Scan or take a picture of the forms and upload them during the application
(just before you sign on the final page) or have your student turn it in to the School.
